How to Complete Patient Portal Forms on your Cell Phone

  2. Sign in with your 兔子先生 Portal credentials.

  3. Click the menu bar (grey bar with three blue lines).

  4. Click on “Forms”.

  5. Scroll down to “兔子先生 Patient Forms” and complete the following forms: 兔子先生 Medical History, 兔子先生 Privacy Policy Acknowledgement, 兔子先生 Release Authorization, and all other forms related to your visit (兔子先生 Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

  6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
